I understand that I will be driving my personal vehicle on behalf of Westwood Baptist Church and carrying additional passengers for their ministry.

I possess a valid Driver's License and I am current on my Motor Vehicle Insurance. (Driver must provide a copy of both to be placed on file for this trip and current year.) A new copy and statement of understanding will be required each new year or if your license expires within this current year.

I also understand that if an accident occurs, while I am driving on behalf of Westwood Baptist Church and carrying additional passengers from Westwood, my Motor Vehicle Insurance will be the primary coverage and Church Mutual Insurance could be a secondary coverage provider.
