My student will be leaving [DNow 2025] early because of a previous commitment. I have completed the information below so my student's host home and small group leaders know when she/ he will leave and when she/ he can be expected to return. I have reviewed the schedule for this event and understand where to return my student.
By signing this form, I am giving my child full permission to leave this event hosted by Westwood Baptist Church. Whether my child is being picked up by a family member or driving themselves, I understand that they are not in any capacity covered by the liability forms or waivers signed to participate in Westwood Baptist Church activities and in no way will Westwood Baptist Church be held liable for any injuries or other catastrophic events that may happen during this time.
